Discharge Planning

Our discharge time is 10 a.m. You may need the support of specific community resources or long-term care once you are discharged from our hospital. Arrangements for these services can take time. It is a good idea to think about what your needs might be following discharge and to begin to make arrangements early on in your stay. You may wish to discuss these with your physician or nurse.

Our Discharge Planning Coordinator may visit with you during your hospital stay to discuss any concerns that you or your family may have regarding your needs once you go home. Our Discharge Planning Coordinator is available Monday through Thursday from 8 a.m. to 4 p.m. by dialing ext. 219.
